This individual shall monitor and manage employee performance and foster interdepartmental communication and education regarding case management activities and roles. Additionally, the Team Lead will work collaboratively with the Manager and Director of Case Management, Medical Staff, and Administration to achieve system/facility key performance indicator (KPI) targets and resource utilization while being acutely aware of facilitating optimal patient outcomes and quality of care within the framework of the mission, vision, and values of Baptist Health. This individual is also responsible for orientation, education, and allocation of human resources in the assigned area. This position may have additional duties assigned that are within scope of the role.**Education / Experience**:Degree in Nursing, Bachelor's degree in Nursing preferred. Two to three years experience in hospital Case Management required with progressive experience in care coordination and/or community transitions of care preferred. Knowledge of community health care and socioeconomic resources.
