Qualifications

• RN license active in good standing.
• Two to three years in hospital Case Management.
• Degree in Nursing; Bachelor's preferred.
• Knowledge of community resources and transitions of care.

Responsibilities

• Coordinate and supervise Case Management for all age groups.
• Monitor employee performance and educate on case management activities.
• Collaborate with Manager, Director, Medical Staff and Administration to meet KPI targets.
• Allocate human resources and oversee orientation in the assigned area.
