Job summary

Baptist Health in Montgomery, AL is seeking an RN - Case Management Team Lead to coordinate and supervise case management across all age groups. This role monitors performance, guides staff, and fosters interdepartmental collaboration to improve patient outcomes in line with Baptist Health's mission.

You will work with the Case Management Manager/Director, allocate human resources, orient staff, and handle education within the assigned area; additional duties may be assigned within scope.

Qualifications

• Bachelor’s degree in Nursing required or preferred.
• Two to three years experience in hospital Case Management required.
• Knowledge of community health care resources.
• Joint Commission and third party payor requirements preferred.

Responsibilities

• Coordinate and supervise the provision of Case Management for all age groups.
• Monitor and manage employee performance; foster interdepartmental communication and education regarding case management activities.
• Collaborate with Manager and Director of Case Management, Medical Staff, and Administration to achieve KPI targets and optimize resource utilization for quality patient outcomes.
