Baptist Health in Montgomery, AL is seeking an RN - Case Management Team Lead to coordinate and supervise case management across all age groups. This role monitors performance, guides staff, and fosters interdepartmental collaboration to improve patient outcomes in line with Baptist Health's mission.

You will work with the Case Management Manager/Director, allocate human resources, orient staff, and handle education within the assigned area; additional duties may be assigned within scope.

• Bachelor’s degree in Nursing required or preferred.
• Two to three years experience in hospital Case Management required.
• Knowledge of community health care resources.
• Joint Commission and third party payor requirements preferred.

Summary

The RN - Case Management, Team Lead shall coordinate and supervise the provision of Case Management for all age groups. This individual shall monitor and manage employee performance and foster interdepartmental communication and education regarding case management activities and roles. Additionally, the Team Lead will work collaboratively with the Manager and Director of Case Management, Medical Staff, and Administration to achieve system/facility key performance indicator (KPI) targets and resource utilization while being acutely aware of facilitating optimal patient outcomes and quality of care within the framework of the mission, vision, and values of Baptist Health. This individual is also responsible for orientation, education, and allocation of human resources in the assigned area. This position may have additional duties assigned that are within scope of the role.

Education / Experience

Degree in Nursing, Bachelor’s degree in Nursing preferred. Two to three years experience in hospital Case Management required with progressive experience in care coordination and/or community transitions of care preferred. Knowledge of community health care and socioeconomic resources. Joint Commission and third party payor requirements preferred.

License / Certification

Active RN license required

Working Conditions/Physical Requirements

• Position requires sitting for 60% of the workday and mobility 40% of the workday.
• There is occasional stooping, bending, kneeling, and crouching.
• There is frequent reaching, use of hands and repetitive action.
• Requires occasional lifting or carrying up to 25 pounds, pushing/pulling up to 150 pounds.
• There is a continuous need for speaking with clear diction and hearing sensitivity in conversation either by phone or in person.
• Continuous visual acuity at close and distant range.
• Inside protected from weather conditions but not necessarily from temperature changes. Minimal noise levels.
• Much exposure to computer work.
• Occasional exposure to risk of electrical shock.
• Some exposure to infectious disease while performing job-related tasks.
• Some exposure to hazardous chemicals. Occasional exposure to radiation.
