Job summary

Qualifications

• Develop and monitor discharge plans with the care team.
• Collaborate with providers to optimize hospital stay length.
• Lead and document care coordination and transitions.
• Educate patients and families regarding care plans.
• Maintain timely case management documentation.
• Identify barriers to discharge and coordinate resources.
