Yale NewHaven Health in New Haven, CT is seeking a Case Manager to coordinate care across inpatient and outpatient settings, ensuring high-value patient outcomes and efficient resource use. The role requires RN licensure in Connecticut and a Baccalaureate degree in a clinically related field.

The Case Manager will work within a multidisciplinary team, lead discharge planning, advocate for patients, and support Medicare documentation and appeals as needed.

Formación

• Minimum of a Baccalaureate degree in clinically related field; RN required.
• Minimum of three years of relevant clinical experience.
• Active RN licensure in Connecticut.
• Knowledge of case management and discharge planning processes.
• Strong communication and collaboration with multidisciplinary teams.

Responsabilidades

• Coordinate and ensure the plan of care within the interdisciplinary health care team.
• Link inpatient care with outpatient services and case management.
• Monitor documentation and variances from clinical pathways or treatment plans.
• Assist clinicians in documenting admissions and continued stays for Medicare notices.
• Develop and implement discharge plans with the health care team and services.
• Ensure continuation of care with appropriate outpatient providers.
• Coordinate referrals to outside agencies and connect patients with community resources.
• Collaborate with leadership to involve clinical nurses in discharge planning.
• Act as a patient advocate and address ethical/legal issues in care.
