Job summary

Hartford HealthCare is seeking a Registered Nurse (RN) for the Discharge Lounge at Hartford Hospital. This role supports patient throughput by coordinating transitions from inpatient units to the Transition Lounge, working with Case Coordination, EMS, and various departments to ensure safe, timely discharge and efficient capacity management.

Required is a Connecticut RN license with experience in discharge planning or care coordination, and strong communication and leadership abilities to guide

Qualifications

• Current CT RN license required.
• Experience in patient flow, discharge planning, care coordination, or case management preferred.
• Strong communication, organizational, and leadership skills.
• Ability to collaborate across multidisciplinary teams in a fast-paced environment.

Responsibilities

• Coordinate transfer of discharge-ready patients to the Transition Lounge.
• Collaborate with unit nurses, Case Coordination, EMS and support services.
• Serve as a central resource for discharge throughput and patient flow initiatives.
• Provide clinical oversight for patients awaiting transportation or post-discharge services.
• Deliver handoff communication to EMS and transportation providers.
• Oversee daily operations and workflow in the Transition Lounge.
• Lead and support Transition Lounge staff to maintain a safe, efficient environment.
• Monitor and address barriers to discharge flow and escalate as needed.
• Ensure compliance with policies, safety standards and regulatory requirements.
• Promote Hartford HealthCare values through professional communication and teamwork.
