Intermountain Health in Colorado is seeking an RN Hospital Care Manager I to deliver comprehensive care management, develop patient-centered plans, and coordinate care across inpatient and outpatient settings.

You will collaborate with patients, families, providers, and payers, using motivational interviewing and data tracking to improve transitions and outcomes. This role emphasizes adherence to regulatory standards and patient-centered care.

Competenze

• Current RN license in state of practice.
• BSN within four years of hire/promotion; degree verification required.
• Inpatient hospital experience with hospital terminology and processes.
• Understanding of disease management and barriers to care.
