Qualifications

• Bachelor’s degree in Nursing required.
• Three (3) years acute care nursing experience; one (1) year acute care case management preferred.
• In lieu of BSN, nursing diploma or ADN may be considered with commitment to an approved BSN/MSN within four years.

Responsibilities

• Coordinate patient care transitions from acute to post-acute settings.
• Collaborate with multidisciplinary care team to align longitudinal care plans.
• Identify social determinants affecting patient outcomes and connect with post-acute resources.
• Navigate regulatory, payer, and care-management processes to ensure appropriate care and timely plans.
