Job summary

MetroPlus is seeking a registered nurse with care management experience to optimize members' health outcomes and delivery of care. You will assess needs, create and implement individualized care plans, and coordinate with a multidisciplinary team to ensure high-quality, cost-effective care.

This hybrid role focuses on transitions of care, medication reconciliation, and addressing social determinants of health.

Qualifications

• Bachelor’s degree required; 2+ years care management experience in healthcare or managed care.
• Ability to read and interpret medical records, claims data, pharmacy, lab reports and prescriptions.
• Ability to work closely with members and caregivers; collaborate with teams.

Responsibilities

• Provide care management support during transitions of care.
• Complete transitions of care assessment to identify member needs.
• Complete medication reconciliation.
• Address members' clinical, psychosocial, financial, environmental, and social determinants of health needs.
• Engage members in a collaborative relationship to support self-management.
