Job summary

Physicians Regional Healthcare System is seeking an experienced Care Manager RN (PRN) to coordinate discharge planning and transitions of care. You will work with an interdisciplinary team to review records, ensure medical necessity, and implement discharge plans that meet patient needs.

Qualifications

Ability to collaborate with patients, families, and interdisciplinary teams.

Responsibilities

Coordinate discharge planning and transitions of care for patients.
Review medical records to assess admission appropriateness and utilization of services.
Collaborate with interdisciplinary teams to ensure effective patient care and resolve transitions of care issues.
Develop and implement discharge plans including post-hospital placement and social services.
Refer cases to physicians or managers when criteria are not met and ensure timely interventions.
Liaise with community agencies to facilitate seamless transitions.
Document case management activities accurately and timely.
