Job summary

Option Care Health, Inc. seeks a Clinical Transition Specialist to educate discharging patients and families on home health infusion options. You will engage hospital staff, physicians, and discharge planners to ensure smooth transitions and timely discharges.

Your role includes coordinating pre-admission info, building referral relationships, and working with Account Managers to meet growth goals while maintaining high-quality patient care.

Qualifications

• Licensed RN or Pharmacist in the state of practice with at least 2 years in healthcare.
• Experience building relationships with stakeholders at multiple levels.
• Ability to educate patients, families, and providers about services and products.

Responsibilities

• Proactively grows referral sources and patient starts.
• Educates patients, families, and staff about Option Care services.
• Coordinate care transitions from hospital to home or ATS with internal/external partners.
• Review medical records for insurance and history; liaise with registration.
• Partner with Account Manager to execute growth plans and promote services.
