Option Care Health, Inc. is seeking a Clinical Transition Specialist to educate patients, families and facility staff about home infusion services and products.

You will place patients in the appropriate alternative site, coordinate with intake, case managers, and discharge planners to ensure a smooth transition and high-quality care. You will build strong relationships with referral sources, partners with Account Managers to grow sales, and guide families through the discharge process to

Qualifications

• RN or pharmacist license in state of practice.
• At least 2 years in healthcare industry.
• Strong relationship-building with stakeholders.
• Experience with discharge planning or home health care is a plus.

Responsibilities

• Grow referrals and educate patients and staff on services.
• Coordinate care transitions to home or ATS.
• Collaborate with Account Managers to expand referrals.
