Job summary

Option Care Health is seeking a Clinical Transition Specialist in the United States to drive transitions from hospital to home/ATS, educating patients and families on services and products, and building strong relationships with referral sources.

You will collaborate with intake, case managers, discharge planners and Account Managers to grow referrals and achieve sales goals, while ensuring high quality of care and seamless patient transitions.

Qualifications

• Experience building relationships with internal and external stakeholders.
• Knowledge of ACHC/URAC standards and applicable healthcare guidelines.
• Strong customer service mindset with focus on quality and satisfaction.

Responsibilities

• Proactively maintains and grows relationships with referral sources to drive sales and patient starts.
• Educates patients, families and facility staff about services and options at alternative sites.
• Coordinate discharge planning and ensure timely patient transitions to home or ATS.
• Collaborate with Account Managers to develop area business and growth plans.
• Communicate with hospital staff and internal teams to gather pre-admission information.
• Arrange for equipment and medical supplies as needed.
• Support education and promotional programs for patients, caregivers and referral sources.
