Job summary

Option Care Health is seeking a Clinical Transition Specialist to educate patients and care teams about home and alternate site infusion solutions. This role focuses on building strong relationships with referrals and hospital staff to drive timely patient discharge and sustained service adoption.

With RN or Pharmacist licensure and healthcare experience, you will collaborate with intake, case management and account teams to coordinate seamless transitions and grow referral partnerships

Qualifications

• Experience building relationships with stakeholders at all organizational levels
• Ability to educate patients, families and facility staff about services
• Familiarity with ACHC, URAC or similar standards and healthcare compliance

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Serves as key contact and representative of Option Care to educate and assist referral sources.
• Interacts with area service providers to sell Option Care services for discharged patients.
• Assesses hospital patients to determine suitability for home or alternate site care.
• Educates patients and caregivers on services to promote timely discharge and therapy independence.
• Coordinates with Account Managers to execute area growth plans and provide feedback on service promotion.
