Job summary

Option Care Health seeks a Clinical Transition Specialist to drive successful patient transitions from hospital to home or alternate sites. You will educate patients, families, and facility staff about services, and proactively build relationships with referral sources to grow referrals and meet sales goals.

The role requires RN or Pharmacist licensure and healthcare industry experience, with travel to care facilities and strong coordination with account managers and clinical teams.

Qualifications

• Experience building relationships with referrals and hospital partners.
• Knowledge of ACHC/URAC standards is preferred.
• Customer service excellence with internal/external stakeholders.

Responsibilities

• Grow referral sources and patient starts.
• Educate patients and families about services.
• Coordinate with hospital staff and internal teams for seamless transitions.
• Support discharge planning and care coordination.
• Travel to care facilities to promote services.
