Job summary

Option Care Health, Inc. is seeking a Clinical Transition Specialist to educate patients, families and facility staff about home infusion services and facilitate transitions to alternate care sites. The role focuses on advancing patient starts by leveraging referrals and internal partners.

You will collaborate with Account Managers and discharge teams to grow referrals, ensure quality of service, and achieve sales goals while supporting compliant, patient-centered care across multiple facilities.

Qualifications

• Must be licensed RN or Pharmacist in state of practice.
• Minimum 2 years in healthcare industry.
• Strong relationship-building and communication skills.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Interacts with area service providers daily to sell Option Care services.
• Educates patients, their families and facility staff about services and products.
• Provides in-person education to patients and caregivers for timely discharge.
• Communicates with agency staff and medical teams to implement treatment plans.
• Gathers pre-admission information and secures required signoffs.
