Job summary

The Cigna Group's Evernorth Workplace Care in Houston, TX seeks a dedicated RN Care Coordinator to support wellness journeys for employees. You will coordinate care, refer to clinical partners, and work with employers and onsite clinics to improve outcomes.

Qualifications

• RN with current unrestricted Texas license and ability to maintain
• Multistate licensure is preferred
• Three or more years of clinical practice experience
• Case management experience is preferred
• Strong oral and written communication skills
• Proficient with MS Office (Outlook, Word, Excel) and general computer skills
• Ability to work in a team environment and adapt quickly

Responsibilities

• Identify and refer customers to eligible programs and care services
• Coordinate care for high-risk patients across providers and onsite clinics
• Serve as a conduit between employer, PCP, and Evernorth resources
• Review patient data and documentation to identify outreach opportunities
• Maintain timely, compliant documentation and information exchange
• Communicate with matrix partners and internal/external programs to support outcomes
• Identify cases for complex case management and refer accordingly
• Collaborate with Evernorth teams for care coordination and data analysis
• Build relationships with local network providers and onsite health centers
• Address gaps in care with providers and community resources
• Foster professional relationships with customers, employers, and care providers
