Evernorth Health Services is seeking an RN Care Coordinator in Houston to support the Evernorth Workplace Care program. You will coordinate care for high-risk patients, refer to clinical partners, and collaborate with employers and onsite health staff to improve outcomes.

The role requires an active Texas RN license, 3+ years clinical experience and strong communication skills. Flexible work options may apply; position is based downtown Houston.

Qualifications

• RN with current unrestricted Texas licensure and ability to maintain licensure.
• Three or more years of clinical practice experience.
• Case management experience preferred.
• Strong oral and written communication skills.
• Proficient with MS Office and healthcare documentation systems.
• Comfortable working in a matrix, cross-functional environment.

Responsabilités

• Coordinate care for high‑risk patients and refer to clinical partners.
• Serve as central liaison between employer, provider, and Evernorth resources.
• Ensure timely documentation and information exchange to support outreach.
• Identify gaps in care and collaborate with providers to close them.
• Maintain professional relationships with customers, employers and care teams.
• Support data analysis and coordination with health management partners.
• Direct non‑clinical customer questions to appropriate resources.
