Responsibilities

• Identify and connect customers to eligible programs within Cigna Healthcare.
• Support care coordination for individual customers as needed.
• Coordinate care for high-risk populations efficiently and cost-effectively.
• Refer to clinical partners and act as a conduit between employers and providers.
• Ensure timely review of actionable data for outreach and engagement.
• Maintain timely, accurate documentation to drive positive clinical outcomes.
• Communicate clearly with matrix partners and internal/external programs.
• Identify cases suitable for complex case management and follow referral processes.
• Build relationships with local network providers to coordinate care.
• Identify and close gaps in care with providers and community resources.
