Job summary

Cigna Healthcare is seeking an RN - Care Coordinator for Evernorth Workplace Care in Cleveland, OH. The role supports care coordination for high-risk populations, collaborates with employers, onsite clinics, and clinical partners, and drives proactive patient engagement.

The ideal candidate has an unrestricted Ohio RN license, 3+ years of clinical practice, and strong communication. Multistate licensure is preferred and a comprehensive benefits package is offered.

Qualifications

• RN with current unrestricted Ohio licensure
• Three+ years of clinical practice experience
• Case management experience preferred
• Excellent communication skills, both oral and written
• Experience working with high-risk patients and in a matrix environment

Responsibilities

• Identify and refer eligible clients to programs within Cigna Healthcare and Evernorth resources
• Coordinate care for high-risk customers and refer to complex/case management as needed
• Serve as central liaison between employer, providers, and Evernorth programs
• Review patient data and reports to identify outreach opportunities
• Ensure compliant, timely documentation and data exchange for outcomes
• Build relationships with local providers and onsite health centers
• Close gaps in care with providers and community resources
• Support coordination of care with data analysis and matrix partners
• Direct customers to appropriate non-clinical resources as needed
