Competenze

RN with current unrestricted state licensure in Ohio
Three or more years of clinical practice experience
BLS/CPR/AED Certification preferred

Mansioni

Identify and refer appropriate customers to eligible programs within Evernorth/Cigna.
Coordinate care for high-risk patients with clinicians and onsite health centers.
Establish and maintain documentation and data exchange for outreach and engagement.
Build relationships with local providers and onsite health resources.
Refer complex cases to specialized case management when needed.
Communicate with partners and customers to support clinical outcomes and cost efficiency.
