Job summary

The Cigna Group is seeking an RN Care Coordinator for Evernorth Workplace Care based in Cleveland, OH. The role focuses on coordinating care for high-risk employees, referring to eligible programs, and partnering with employers, onsite clinics, and clinical partners.

Qualified candidates will hold an Ohio RN license, have 3+ years of clinical practice, and be proficient in Office software. BLS/CPR/AED certification is required; multistate licensure is a plus.

Qualifications

• RN with current unrestricted state licensure in Ohio and ability to maintain
• Multistate licensure is preferred
• Three or more years of clinical practice experience
• Case Management experience is preferred
• High energy with excellent oral and written communication skills
• Current BLS/CPR/AED Certification

Responsibilities

• Identifies and refers appropriate customers to eligible programs within CIGNA Healthcare and all eligible client specific health and wellness programs
• Supports Care Coordination of the individual customer and coordinates care for high-risk populations with appropriate clinical partners
• Acts as central conduit between employer, provider and Evernorth programs and resources
• Maintains timely, compliant documentation and data exchange to drive outreach and engagement
• Communicates clearly with matrix partners and refers to internal and external programs to support outcomes
• Identifies cases for complex case management and follows referral processes
• Collaborates with Evernorth internal team to support coordination of care and data analysis
• Identifies gaps in care and coordinates with providers or local resources to close gaps
