What You'll Do

• Identify and refer customers to eligible programs within Cigna Healthcare and client-specific health and wellness programs.
• Support care coordination for high-risk targeted populations efficiently and cost-effectively.
• Partner with employers, providers, and Evernorth programs to facilitate care.
• Review patient-level actionable data to identify customers for outreach and engagement.
• Maintain timely and compliant documentation to drive outreach and positive clinical outcomes.
• Communicate with matrix partners and refer to internal and external programs.
• Identify cases for complex case management and follow referral processes.
• Build relationships with local network providers and refer to onsite health centers.
• Identify gaps in care and collaborate with providers and community resources to address them.
• Coordinate with health management partners for additional engagement opportunities.

Requirements

Education:

• RN with current unrestricted state licensure in Ohio

Experience:

• 3+ years of clinical practice experience
• Case management experience preferred

Licenses & Certifications:

• Current BLS/CPR/AED Certification required
• Multistate licensure preferred
