Job summary

VCU Health is seeking an experienced RN Care Coordinator to support patient transitions from acute care to post-discharge settings. You will work with physicians, social workers, and discharge planning to ensure efficient, safe, and cost-effective care.

The role emphasizes collaboration across teams, adherence to CMS guidelines, and active participation in patient safety initiatives within a values-driven healthcare system.

Qualifications

• Must be licensed RN in the State of Virginia or eligible.
• Minimum of three years of nursing experience in an acute care setting.
• Bachelor's degree in Nursing or a related field; Master's preferred.

Responsibilities

• Collaborate with attending and consulting physicians to facilitate transition through hospitalization.
• Work with the multi-disciplinary team to coordinate care across the continuum and ensure reimbursement by payers.
• Engage social workers to resolve psychosocial issues and develop complex discharge plans.
• Serve as liaison between hospital and community agencies for information exchange.
• Maintain compliance with QIO and CMS guidelines; assess medical necessity and assist in appeals.
• Participate in patient safety initiatives and uphold VCUHS Mission and Values.
• Establish and maintain professional rapport with providers, patients, and internal customers.
