Qualified candidates hold Virginia RN licensure (or eligibility), with a BSN or higher and 3+ years in acute care.

Qualifications

• RN licensure in Virginia or eligible for licensure
• Annual requirement: 15 continuing education units
• Minimum 3 years of nursing in an acute care setting
• Experience in UM/care coordination preferred
• Education: BSN or equivalent (nursing or health sciences)
• Master's in Nursing or healthcare field preferred

Responsibilities

• Collaborate with attending and consulting physicians to facilitate efficient hospitalization transitions
• Coordinate care with multidisciplinary team to meet patient needs and ensure appropriate reimbursement
• Engage social workers to resolve psychosocial issues and develop discharge plans
• Maintain compliance with QIO and CMS guidelines and participate in patient safety initiatives
