• Seeks social work expertise to resolve psychosocial care issues and develop discharge plans when needed.
• Serves as liaison between hospital and community facilities for information exchange.
• Maintains compliance with CMS guidelines and QIO-related requirements.
• Assists in evaluating care level and setting using medical necessity criteria and supports safety initiatives.
• Upholds VCUHS Mission and Values in all responsibilities.
• Establishes professional rapport with providers, patients, families and internal customers.
