• Graduate of an accredited school of nursing and minimum two years of acute hospital clinical experience.
• Master's in Case Management or Nursing with at least one year of experience.
• Registered Nurse: WA license upon hire.
• Basic Life Support (CPR) within 90 days.

Responsibilities

• Completes discharge planning assessment and updates plan as needed.
• Facilitates a multidisciplinary discharge plan with patient and post-acute providers.
• Oversees and evaluates discharge plan implementation.
• Collaborates to ensure appropriate inpatient resource utilization using guidelines.
• Coordinates transitions to post-acute services based on clinical needs.
• Identifies readmission risk and implements mitigation for at least 30 days.
