Job summary

VCU Health is seeking a RN Care Coordinator to join a multidisciplinary team in Richmond, VA. You will drive safe, cost-effective care from admission through post-discharge, coordinating with physicians, social workers, and payers to optimize patient outcomes.

The role requires Virginia RN licensure (or eligibility) and at least three years of acute care nursing experience; a Master's in Nursing is preferred. Day and evening shifts are available, with opportunities to impact care continuity.

Qualifications

• Licensed Registered Nurse in the State of Virginia or eligible to practice
• Minimum of three years of nursing experience in an acute care setting
• Bachelor’s degree in Nursing or a related biological/health science field from an accredited school
• Master’s degree in Nursing or a healthcare related field from an accredited program (preferred)
• Experience in UM/care coordination or academic healthcare (preferred)

Responsibilities

• Collaborate with attending and consulting physicians to facilitate transitions through hospitalization.
• Coordinate with multi-disciplinary team to ensure patient needs are met and care is reimbursed by payers.
• Consult with social workers to resolve psychosocial issues and develop complex transition/discharge plans.
• Serve as liaison between hospital and community agencies for information exchange.
• Ensure compliance with QIO series and CMS guidelines and participate in patient safety initiatives.
• Establish and maintain professional rapport with providers, patients/families, and internal customers.
