Kelsey-Seybold Clinic, part of the Optum family, seeks an ED RN Case Manager to monitor ADT notifications and coordinate transitions of care with hospitals and outpatient resources. Location Pearland, TX with remote work flexibility.

Requires BSN, current RN license, 4+ years acute care, knowledge of utilization management and discharge planning; ED or managed care experience preferred. Salary range $60,200–$107,400; benefits and equity options included.

Competenze

• RN license (Multi-state) in good standing
• 4+ years of diverse clinical experience caring for acutely ill patients
• Knowledge of utilization management, quality improvement and discharge planning
• Emergency Department experience and managed care/case management preferred

Mansioni

• Monitor real-time ADT notifications to provide proactive patient and provider outreach and discharge coordination.
• Coordinate discharge planning and transitions of care with hospitals, physicians and outpatient resources.
• Support ED discharge follow-up care per approved clinical guidelines to ensure continuity of care for members.
