Job summary

Kelsey-Seybold Clinic, part of Optum, is seeking an ED RN Case Manager in Pearland, TX. You will monitor real-time ADT notifications and coordinate discharge planning to support seamless transitions of care across hospitals, PCPs, specialists, and home care teams.

The ideal candidate holds an undergraduate nursing degree, an unrestricted RN license, and at least four years of diverse clinical experience. ED and managed care/case management experience are preferred. Remote work options may apply.

Qualifications

• Undergraduate nursing degree.
• Current, unrestricted Multistate RN license.
• 4+ years of diverse clinical experience caring for acutely ill patients with multiple disease conditions; knowledge of utilization management, quality improvement, and discharge planning.
• Emergency Department (ED) experience; managed care and/or case management experience.

Responsibilities

• Monitors real-time ADT notifications to provide proactive patient and provider outreach and discharge coordination.
• Supports ED discharge follow-up care and transitions of care in collaboration with hospitals, physicians, and outpatient resources.
