Formación

• RN licensure in state where practicing.
• Bachelor of Science in Nursing (BSN) or 5+ years of case management experience in lieu of BSN.
• Experience in remote/telephone-based case management.

Responsabilidades

• Coordinate referrals from physicians and facilities for high-risk members.
• Educate members about illnesses, planned treatments, and care plans.
• Develop and maintain individualized Member Care Plans with physicians.
• Participate in clinical improvement activities to enhance care coordination.
• Facilitate communication among care team and members.
