AccentCare, Inc. in Minnesota is hiring an RN Case Manager to manage patient care plans from admission through discharge, directing staff and coordinating disciplines to deliver quality care. The role combines direct and indirect patient care and follows the patient’s plan of care.

Schedule is Monday–Friday, 8:00 a.m.–5:00 p.m. The position offers a sign-on bonus of $5,000, with pay estimated at $87,000–$102,000 per year, and compensation described as annualized though based on per-point

Competenze

• Graduate from an approved school of professional nursing and licensed to practice as an RN in the state of operation.
• Minimum of one year of RN experience.
• Current CPR Certification required.

Mansioni

• Assess the status of nursing needs and manage the patient’s care plan.
• Complete initial patient assessment upon admission with patient history.
• Select appropriate nursing diagnoses and establish measurable goals.
• Review medications for potential interactions or adverse effects.
• Inform physician of assessment results and recommended interventions.
• Implement nursing interventions within the Nurse Practice Act.
• Re-evaluate patient needs as required.
• Document assessments, progress and discharge notes timely.
