Geisinger Cancer Center Dickson City is seeking an RN Coordinator Cancer Care to assess patient needs, plan and provide nursing care, and coordinate follow-up in outpatient Hematology/Oncology. This full-time role emphasizes care navigation, patient education, and triage across the continuum of care.

You will coordinate treatment plans, support the care team, and ensure timely communication of results and changes to patients, families, and providers.

Competenze

• RN with at least 2 years of nursing experience required.
• BLS certification required; chemotherapy/biotherapy certifications within 3 months of hire.
• Licensed RN in Pennsylvania; ONCC certification required after required hours.

Mansioni

• Coordinate treatment plans for Hematology/Oncology patients across the continuum of care.
• Provide direct patient care and education in the Hem/Onc department.
• Serve as a resource for patients and families regarding diagnoses and care plans.
• Act as a liaison between patient and care team to address changes in treatment.
• Perform pre-visit RN triage to support timely decisions and planning.
• Assist with referrals to supportive care such as Palliative, Psychology, and Social Work.
