Job summary

Frederick Health is seeking a fulltime Registered Nurse Cancer Patient Navigator for Oncology and Hematology office Monday to Friday 8am to 4:30pm.

The Oncology Nurse Navigator coordinates patient care along the cancer journey, provides education, and serves as a resource to patients, families, and the healthcare team. This role supports access to survivorship services and collaborates with physicians, nurses, social services, and community resources.

Qualifications

• RN licensed to practice in Maryland.
• BSN degree required.
• 3–5 years RN experience in hospital or clinic; oncology experience preferred.
• OCN certification preferred or 12 CEUs with oncology focus within first year; Chemotherapy/biotherapy certification from ONS preferred.
• Maintain licensure and yearly CEU requirements for OCN and RN.
• CPR certification required.
• Ability to work in a self-directed environment and lead teams.
• EMR experience preferred.

Responsibilities

• Assist patients through their cancer journey from consultation to survivorship.
• Ensure timely diagnosis and treatment through coordination and communication.
• Serve as liaison between Oncology/Hematology providers and referring physicians.
• Identify and remove barriers to care; facilitate referrals for support services.
• Provide education and information to patients and designated individuals.
• Participate in cancer consultations, treatment planning, and restaging visits; document care plans and teaching.
• Respond to patient requests for information regarding disease, side effects, and resources.
• Guide support staff regarding care plans and appointments; ensure efficiency and completeness.
• Track survivorship appointments and related materials and referrals.
