Job summary

The Cancer & Hematology Centers are seeking a Nurse Navigator to coordinate and guide patients through the cancer care continuum, offering education, triage, and care coordination. This RN role ensures patients understand diagnosis, treatment options, appointments, financial resources, and support services.

Based at our LHCP clinic, the position runs Monday to Friday with no weekends or holidays. A $2,000 sign-on bonus is included to welcome you to the team.

Qualifications

• Must be a licensed Registered Nurse in Michigan.
• Oncology Certification or within 1 year of employment.
• Chemotherapy/Biotherapy Provider Card or within 6 months of employment.
• MICMT Training or within 60 days of employment.
• BLS certification or within 90 days of employment.
• Successful completion of orientation program.

Responsibilities

• Function as primary clinical contact for patients and families.
• Coordinate diagnostic work-up, treatment starts and follow-up visits across ambulatory oncology services.
• Collaborate with physicians, infusion nurses, pharmacists, social workers, dietitians, and community resources.
• Provide patient education related to diagnosis, treatment plan, chemotherapy/immunotherapy, symptom reporting, side effect management, and clinic workflows.
• Support timely prior authorization, referrals, outside record retrieval, and coordination of tests or procedures to avoid delays.
• Identify and help address barriers to treatment, including transportation, finances, scheduling, language access, and psychosocial needs.
• Track patient progress through ambulatory milestones and document navigation interventions in the EHR.
• Perform telephone triage for urgent symptoms and escalate to the appropriate provider.
• Participate in care management to coordinate follow-up, support high-risk patients, and ensure team collaboration.
• Partner with Nursing Leadership to perform quality improvement activities and optimize workflows.
