Job summary

Warren Clinic, Inc. seeks a full-time nurse care coordinator to collaborate with physicians and staff in delivering patient-centered medical home and care coordination within the Tulsa office setting.

You will oversee disease registry activities, perform pre-visit chart reviews, and support goal-attainment for blood pressure and glucose control through 5A’s self-management planning and patient education.

Qualifications

• RN license with Oklahoma license and BLS certification.
• 3–4 years of outpatient medical group practice experience.
• Demonstrates effective teaching and adult learning principles.
• Ability to coordinate educational materials for patients and families.
• Professional communication with patients, families and staff.

Responsibilities

• Oversees the disease registry database and tracks overdue visits.
• Identifies patients not meeting clinical goals and arranges follow-up.
• Pre-visit chart review for preventive care and chronic disease interventions.
• Prepare forms and communicate reviews to providers.
• Coordinate care across the continuum and liaise with hospital and specialists.
• Support self-management plans and patient education.
