Job summary

Central City Health is seeking a Nurse Care Coordinator to oversee a panel of adult members, ensuring continuity of care across primary and behavioral health services. The role requires a Michigan RN license and BLS certification, with 2–3 years of ambulatory care or care coordination experience preferred.

Responsibilities include proactive registry-driven care, medication reconciliation, and coordinating with pharmacists, case managers, and dental providers to ensure timely interventions.

Qualifications

• Active, unrestricted Michigan RN licensure required.
• BLS certification required.
• Experience with ambulatory/primary care and care coordination preferred.
• Experience with EHR registry/panel management.
• Ability to triage and perform medication reconciliation.

Responsibilities

• Own a panel of CCV-eligible adult members and provide continuous oversight.
• Monitor EHR for qualifying events between visits and coordinate care.
• Perform medication reconciliation and coordinate pharmacist consults.
• Triage CCV/ISQ findings needing escalation with providers.
• Coordinate hand-offs to Case Manager, Pharmacist, or Dental providers as needed.
• Lead monthly multi-disciplinary rounds based on clinical criteria.
• Serve as clinical point of contact between CCVs for the adult population.
