Job summary

Qualifications

• Graduate from an accredited school of Professional Nursing (Diploma, ADN, BSN).
• Current state licensure as a Registered Nurse (RN).
• BLS certification through AHA or Red Cross.
• Three to five years of nursing experience.
• Preferred: BSN, specialty certification, outpatient/hospital experience, ED experience, ACLS, care coordination experience.

Responsibilities

• Collaborate with a multidisciplinary care team to coordinate care for high-risk patients across the continuum.
• Provide care and disease management coordination and targeted outreach.
• Outline involvement needed by the specialty care team and primary care team.
• Utilize assessment tools to identify care needs and manage patient populations.
• Educate patients and families, monitor progress, and update care plans as needed.
• Serve as liaison and advocate for patients and families, ensuring smooth transitions of care.
