Job summary

Azza HealthCare Agency seeks an experienced nurse to join our clinical team in promoting preventative care, conducting screenings, and coordinating disease management services. You will provide primary clinical support to providers, patients, and families, assisting in diagnosis and treatment planning.

You will assess needs with patients and families, educate on disease management, coordinate tests, and help develop and follow care plans.

Qualifications

• RN with at least 2 years of clinical experience.
• Graduate from a specialty nursing program; bachelor’s degree preferred.
• Strong clinical, communication, and patient education skills.
• Ability to collaborate effectively within interdisciplinary teams.
• Proficiency in clinical documentation and diagnostic coordination.

Responsibilities

• Assess healthcare, educational, and psychosocial needs with patients and families to set treatment goals.
• Coordinate communication among healthcare team members to address barriers and options.
• Educate patients and families on disease management and care programs.
• Implement clinical interventions based on risk assessments and evidence-based practices.
• Coordinate laboratory and diagnostic testing with accurate documentation and follow-up.
• Support post-discharge care coordination and develop individualized care plans.
• Administer medications and vaccines within scope of practice.
• Maintain accurate records in electronic systems and participate in professional development.
