Job summary

Utica Park Clinic in Tulsa, OK is seeking a day shift, full-time Clinical Navigator (RN) to coordinate and optimize patient care across the continuum. The role functions as a clinical resource and patient advocate, collaborating with families, providers, and interdisciplinary teams to ensure excellent outcomes and a positive experience.

The Clinical Navigator RN supports care coordination, patient education, and program initiatives while adhering to evidence-based standards.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license.
• BLS certification within 14 days of hire or transfer.

Responsibilities

• Coordinate and facilitate patient care across the continuum for designated patient populations.
• Serve as a clinical resource, patient advocate, and liaison among patients, families, providers, and interdisciplinary teams to promote high-quality outcomes and a positive patient experience.
• Support care coordination, patient education, clinical program initiatives, and evidence-based practice standards.
• Contribute to improved clinical outcomes through effective communication, collaboration, and patient-centered care management.
