Qualifikationen

• Bachelor's Degree in Nursing from an accredited program.
• Non-BSN RN may be considered if pursuing a Bachelor's within 2 years of hire.
• Minimum two years' experience in acute care.
• Three to five years' experience in care management, utilization review, home care and/or discharge planning.
• Licensed registered nurse (RN) in Michigan preferred.
• Case Manager, Certified (CCM) preferred.

Aufgaben

• Identifies patients needing care management services (utilization review, care coordination, discharge/transition planning).
• Manages a caseload of patients for utilization management and care coordination during stay.
• Plans safe and effective discharge and transition to appropriate post-hospital level of care.
• Communicates with payers, patients/families, physicians, and the care team to coordinate care.
