Job summary

The Valley Hospital in Paramus is seeking a Clinical Care Coordinator, Case Manager to coordinate patient progression, discharge planning, utilization management, and transitions of care across the continuum.

The role requires an RN with a BSN, NJ licensure, and five years of clinical nursing experience; ANCC Case Management Certification is preferred. Collaboration with physicians, families, and payers is essential to ensure optimal outcomes.

Qualifications

• Requires BSN in nursing and RN licensure in NJ.
• ANCC Case Management Certification preferred, experience in case management and utilization review is desirable.
• Minimum five years of clinical nursing experience required, with preferred exposure to medical/surgical, perioperative, procedural, or emergency nursing.
• Strong knowledge of care coordination, discharge planning, CMS regulations, and healthcare reimbursement.

Responsibilities

• Coordinate patient progression, utilization management, discharge planning and transitions of care across the continuum.
• Collaborate with patients, families, physicians, payers and interdisciplinary teams to promote appropriate resource utilization.
• Review admission status and ensure timely patient placement and regulatory compliance.
• Maintain professional standards in a fast-paced hospital environment.
