Corewell Health is seeking a part-time Care Manager to integrate cost, quality and utilization in patient admission, stay, and discharge planning at the Trenton site. The role emphasizes coordination of care, utilization review, and collaboration with the care team and payers to optimize outcomes and reimbursement.

The position requires an RN with a Bachelor's in Nursing (or actively pursuing a BSN within 2 years) and 3–5 years in care management, UR, home care or discharge planning.

Qualifications

• Bachelor's Degree in nursing is required.
• RN license required; those pursuing BSN within 2 years may be considered.
• 3–5 years in care management, utilization review, discharge planning is preferred.

Responsabilités

• Identify patients needing care management and utilization review.
• Manage caseload for care coordination during hospitalization and discharge planning.
• Coordinate transitions to appropriate post-acute care and levels of care.
• Collaborate with physicians, payers, and the care team to obtain authorization and ensure appropriate reimbursement.
• Contribute to improving discharge planning and care transitions.
