University Hospital in Newark is seeking a Case Management Coordinator to guide a patient’s journey through the care continuum, collaborating with physicians and clinical staff to ensure appropriate, cost-effective utilization and timely discharge planning.

The role emphasizes regulatory compliance, patient-centered transitions of care, and partnership with the care team to optimize outcomes and resource use within covered benefits.

Qualifications

• RN license in NJ is required.
• Bachelor’s in Nursing or healthcare; Master’s preferred.
• 4+ years of acute care hospital clinical experience; case management certification preferred.
• Experience in case management, home health, utilization review; basic computer skills.
• Weekend rotation and strong communication skills expected.

Responsibilities

• Coordinate multidisciplinary plan of care for patients with complex needs and arrange discharge planning.
• Identify level of care, anticipate length of stay, and ensure compliance with health regulations.
• Develop and implement case management plans and interventions; coordinate transfers and authorizations.
• Collaborate with Social Work and utilization management to reduce LOS and prevent denials.
• Contribute to safety, infection control, and quality improvement initiatives.
