Job summary

CentraState Medical Center in Freehold, NJ is seeking a Care Coordinator to manage patient care from pre-admission through discharge, coordinating resources to ensure high-quality service and favorable financial outcomes. The role requires a Bachelor's in Nursing, at least 2 years of Acute Care RN experience, and an active NJ RN license.

Knowledge of discharge planning, utilization review, and quality improvement is preferred.

Qualifications

• Bachelor's Degree in Nursing required.
• 2+ years of Acute Care RN experience.
• Current NJ RN License.
• Knowledge of utilization, quality improvement, case management, or discharge planning.
• Computer literacy preferred.
• Strong professional, organizational, and interpersonal skills.

Responsibilities

• Provides education to patient/family regarding disease processes and discharge plans.
• Reviews hospital records to determine medical necessity for admission and continued stay.
• Utilizes effective communication with patients, family, physicians, nurses, staff and others to achieve outcomes.
• Plans, organizes and prioritizes work to impact clinical, financial and service outcomes.
• Facilitates post-hospitalization discharge needs and coordinates with social worker.
• Maintains communications with services to ensure quality care and timely discharge planning.
• Collects and documents accurate data.
• Assesses and identifies patients needing Case Management in ED.
• Collaborates with team for bed assignment and admission through Interqual criteria.
• Develops initial discharge plans for ED admitted and observation patients.
• Offers alternative care settings when needed.
• Participates in performance improvement in ED for optimal outcomes.
