Job summary

Corewell Health in Trenton, MI is seeking a Part-time RN to lead care management and discharge planning for hospitalized patients. You will coordinate admissions and transitions, review medical necessity, and work with payers and the care team to ensure quality, cost-effective care.

Responsibilities include managing a patient caseload, facilitating utilization management, and supporting safe discharge planning in a 20-hour-per-week schedule. A Michigan RN license and CCM/BLS are preferred.

Qualifications

• Bachelor's degree from an accredited nursing program.
• Active Michigan RN license; BSN preferred or pursuing within 2 years.
• Two years in acute care; three to five years in care management preferred.
• BLS certification (AHA or ARC) preferred; CCM certification preferred

Responsibilities

• Identifies patients needing care management services (utilization review, care coordination, discharge/transition planning).
• Manages a caseload to facilitate utilization management and care coordination during stay and transitions.
• Uses critical thinking to determine alternative care courses and supports readmission reduction initiatives.
• Reviews cases for medical necessity and coordinates with physicians, payers, and staff to obtain authorization for care.
• Communicates with payers, patients, families, and the care team to coordinate care and transitions.
• Documents reviews, plans, and interventions in accordance with policies and regulatory requirements.
• Collaborates with departments to identify inefficiencies and participates in improvement projects.
