Qualifications

• RN license in Michigan required.
• BLS certification preferred or active.

Responsibilities

• Identify patients needing care management (utilization review, care coordination, discharge planning).
• Manage caseload to facilitate utilization management and safe discharge/planning.
• Collaborate with physicians, payers and care teams to obtain authorization and ensure appropriate level of care.
• Communicate with patients/families to coordinate post-acute transitions and follow-up.
