Job summary

BoldAge PACE is seeking an RN Care Manager to assess participants, provide nursing interventions, and evaluate care outcomes across clinic, center, and home settings. You will work with an interdisciplinary team to develop plans of care tailored to each participant, ensuring timely follow-up and coordination of services.

The role requires nursing licensure, BLS within 90 days, and experience with frail or elderly populations.

Qualifications

• Bachelor's degree in Nursing preferred.
• State RN license required.
• BLS required (within 90 days of employment).
• 1 year of experience with frail or elderly population preferred.

Responsibilities

• Provide high quality clinical care as a member of the IDT.
• Coordinate home care services according to each participant's plan of care.
• Participate in 24/7 on-call triage for participants' needs.
• Assess, plan, and coordinate participants’ home care services.
• Collaborate with primary care providers and clinic staff.
