The Information Center is seeking a Supports Coordinator/Registered Nurse to work under the Clinical Manager in Wayne County (Out-Wayne). The role focuses on assessing needs across medical, functional, psychological, financial, and environmental areas and coordinating with care teams.

The position requires Michigan RN licensure, strong collaboration with service providers, and ability to travel within a defined region. Non-Exempt, full-time status with department supervision support.

Qualificações

• Current license as Registered Nurse in Michigan.
• Prefer two years in hospital, home care, or community setting.
• Valid Michigan driver’s license and reliable transportation; ability to travel within Out-Wayne County.

Responsabilidades

• Assess disease progression and coordinate with hospice, skilled care, and community mental health services.
• Develop and update the Individualized Person-Centered Service Plan with participants and supports.
• Conduct in-home reassessment visits with RN/SW team and document activities.
• Link participants to community services and supports; involve family members and informal caregivers.
• Maintain documentation within required timeframes and ensure licensure compliance.
